A Common Problem, Rarely Discussed

Leaking a little urine when you cough, sneeze, laugh, lift or exercise, or feeling a sudden urge you can't hold, is very common. It is more common in women, especially after childbirth and around menopause, but men get it too. You don't need to feel embarrassed. It can often be improved.

Ayurveda sees urinary incontinence mainly as a Vata disorder caused by weak bladder and pelvic muscles.

Step 1: Strengthen the Pelvic Muscles

Ashwini Mudra / Pelvic Floor Exercise

  1. Sit comfortably.
  2. Gently squeeze and lift the muscles around your back passage and the muscles you would use to stop urine flow.
  3. Hold for 3 to 5 seconds, then relax fully.
  4. Do 10 to 12 repetitions, 2 or 3 times a day.

This is very similar to the Kegel exercise recommended by doctors. Results usually show after a few weeks of regular practice.

Don't practise by stopping your urine midstream. Do it when your bladder is empty.

Helpful Yoga Poses

  • Vajrasana (sitting on your heels)
  • Gentle forward bends
  • Bridge pose (Setu Bandhasana)

Step 2: Train Your Bladder

Some people go to the toilet "just in case" every time they see one. Over time, the bladder gets used to holding less. Instead:

  • Try to go at regular times, for example every 2 to 3 hours.
  • When you feel an urge, wait a few minutes and gradually stretch the gap.

Step 3: Food and Drinks

  • Cut down on tea, coffee, cola and alcohol. They make you pass urine more often and increase urgency.
  • Do not stop drinking water. Too little water makes urine strong and irritates the bladder. Just spread your water intake through the day and drink less in the 2 hours before bed.
  • Follow a warm, Vata-calming diet and avoid constipation, which puts pressure on the bladder.
  • Eat a small handful of white sesame seeds with a little jaggery daily. Ayurveda uses this to strengthen the bladder. People with diabetes should skip the jaggery.
  • Keep a healthy weight, which reduces pressure on the bladder.

About Herbs

Ayurvedic practitioners may use strengthening herbs like Ashwagandha, Bala and Vidari. Take them only under guidance.

When to See a Doctor

  • Burning, pain, or strong-smelling or cloudy urine (possible urine infection)
  • Blood in the urine
  • Leakage that starts suddenly, with back pain, leg weakness or numbness around the private parts. This is an emergency.
  • Men with a weak stream, dribbling or getting up often at night (check the prostate)
  • Leakage that affects daily life, or continues months after childbirth. A doctor or pelvic physiotherapist can help a lot.
